In CTech’s experience, one of the major areas of Provider discontent in transitioning
to EHR’s is the time it takes to document a Clinical Episode. EHR’s
are particularly strong in using Template Based Documentation strategy but can only
capture certain portion of the documentation easily. EHR’s come up somewhat
short when it comes to capturing critical “episode-specific” contextual
information that is so vital to document for proper patient care and safety and
even billing appropriately. The alternative is to have the Provider use “free
text” data entry for which they do not have the time and CTech believes that
is not the right use of a Provider’s time in the first place. CTech has developed
an approach that combines the advantages of EHR with narrative form of documentation
(dictation method is widely used and still the fastest method to document).
Voice Capture Solution - Key Attributes:
- Works very well with "Templates Based Documentation systems that support embedded
voice capture"
- CTech's system is capable of receiving one or multiple "voice files" for a single
"Voice Capture Episode". These files are identified through Control ID (robust in
uniqueness) provided by the Sending Application and these files are returned to
that Sending Application with the same Control ID so that the transcribed information
is merged appropriately with the rest of the documentation for that Clinical Episode
- Some of the major advantages of this delivery model are:
- No need for transmission of Protected Health Information (PHI) to the transcription
vendor. This greatly enhances the HIPAA compliance of the overall MT process
- This approach allows the Providers to use “Template Based Documentation”
when appropriate and to use “Embedded Voice” for sections of the Voice
Capture that requires such flexibility (i.e. History of Present Illness, Assessment
and Plan etc.)
- Works well as a Middleware solution between hospital EHR and transcription service
provider
- Cost savings of 40-50% compared to 100% traditional “narrative/dictation”
method
- Rapid playback of untranscribed voice data from within the document is instrumental
in improving patient care for rounding and other physicians
- Saves Physicians time by not requiring dictation of PHI data
This approach also allows for accurate billing for the transcribed work. The client
gets billed only for the portion of the document that is transcribed. In addition,
this approach also allows for accurate monitoring of the utilization of the Template
Based Approach of Voice Capture by various Providers.
